Description	Offers a variety of services to help caregivers care for their loved ones. Helps with case management, referrals, and linkage. Supportive services are available, such as caregiver respite, providing temporary assistance to help care of loved one in home, or with in-home assistance with chores or homemaker tasks.




Hours Of Operation	Office closed on New Year's Day, Martin Luther King Jr's Birthday, Washington's Birthday, Memorial Day, Independence Day, Labor Day, Veteran's Day, Thanksgiving Day and the day after, and Christmas Day Mon: 8am 5pm Tue: 8am 5pm Wed: 8am 5pm Thu: 8am 5pm Fri: 8am 5pm

Eligibility	Must be a caregiver and meet the following criteria: Be 18 years of age or older Provides in-home care to an individual 60 years of age or older and frail *Lives in San Joaquin County
