Description HEALTH CARE: Provides primary care services but not limited to women's health, optometry, diabetic eye screening, audiology, hearing aid fitting, podiatry and nutrition education. Also offers mental health services and stop smoking program. 

VA BENEFITS ELIGIBILITY & FEES:
ELIGIBILITY: All Veterans are encouraged to enroll for care, and it costs nothing to enroll. Eligibility is dependent on the following factors:
  • Nature of a Veteran's discharge from military service (e.g. honorable, other than honorable, dishonorable)
  • Length of service
  • Va service-connected disabilities; and income level.

FEES: Free for veterans with service-connected injuries and veterans who earn below the poverty income level.
