HOUSING NAVIGATION: Works with individuals/families experiencing homelessness and meets Category I (identifies as living on the street/unhoused for up to 9 months) or Category 4 (individuals fleeing domestic violence situation) criteria. Completes assessment for Coordinated Access Intake to determine eligibility for housing resources and connects to MOHS for housing placement. May offer case management services.

Program
Eligibility Individuals or families meeting Catergory 1 or 4 critieria for homelessness. Category 1: Literally Homeless Individual or family who lacks a fixed, regular, and adequate nighttime residence, meaning: Has a primary nighttime residence that is a public or private place not meant for human habitation. Homeless Category 4: Fleeing/Attempting to Flee Domestic Violence Any individual or family who: Is fleeing, or is attempting to flee, domestic violence; Has no other residence; and Lacks the resources or support networks to obtain other permanent housing
Program Fees None.
Application Process Walk-in on Monday, Wednesday, or Thursday, from 8:30AM-11:30AM. It is recommended for people to arrive early, between 8:30AM-9:15AM, because only a set amount of people can be screened per day.
Documents Required ID, Social security card, and birth certificate if available.
