HOME CONNECTIONS: Offers the Housing First program for chronically homeless individuals by Referral Only. Provides permanent housing with supportive case management. Residents live in private homes scattered throughout the community located in neighborhoods that match the needs of each individual.

Eligibility
|
Referral Required. Families and individuals experiencing chronic homelessness, with a documented physical disability and referral from a Physician, Social Worker, Health Care Professional, etc.

*Clients are only accepted through Coordinated Access. MUST first complete a case management intake at Beans and Bread or any program that has coordinated access navigators on site.
