Offers an integrated team with year-round care, including counseling services and medication therapy. Manages the clients physical needs, addresses housing and employment issues, and provides other services.

Eligibility
|
Residents of Baltimore City 18 years and older with chronic mental illness or dual diagnosis. Accepts individuals who have been hospitalized in a state psychiatric facility for 6 or more consecutive months; or have been in an inpatient psychiatric unit at least 4 times within the past two years; or have been at the ER at least 7 times in the past two years for any psychiatric reason.

Program Fees
|
The Maryland Department of Health and Mental Hygiene supports Chesapeake Connections, which means there are no fees for the services provided.
