Applications will not be reviewed until all requirements are met. • DD214 Member-4 copy showing Discharge Status • MD Driver's License or State ID (birth certificate/social security card) of Applicant, Spouse/Partner and Dependent(s) • 2 months of Bank Statements • The last 30 days of paystubs or Unemployment Statements • VA Award Letter • Letter of Circumstance • Expense Invoice or Statements as proof of need
