Description	UTILITY ASSISTANCE: NOTE: THERE ARE A LIMITED NUMBER OF APPOINTMENT SLOTS AVAILABLE EACH WEEK * 60 YEARS OF AGE OR OLDER OR MUST BE ON SSI DISABILITY * FOR HOUSTON COUNTY ONLY: Must call for an appointment during normal Salvation Army work hours anytime during the week, Mon-Thu 9-3, closed one hour for lunch. Fri 9-12. These are Project Share funds so there is an age restriction of 60 years of age or older OR must be on SSI Disability; however, utilities must be in the name of the person requesting assistance and live in Henry County.

Documents Required	Please note they will be completing the applications for assistance via phone and email. Please be sure to have eligibility verification documents ready and able to email, scan or fax them to our office. If unable to electronically send the required information, a time and date will be determined during the phone intake to bring the documentation to the service window. FINANCIAL ELIGIBILITY/PROOF OF INCOME Examples • Social Security award letter • Actual check/ earning statement • Print-out from Social Security • Bank statement showing direct deposit for qualifying individual Other Documents needed • Valid photo ID • Social Security card • Proof of residency in Houston or Geneva County • Copy of current unpaid utility bill (disconnect notice will not be accepted) • UTILITY BILL MUST BE IN THE NAME OF THE HEAD OF HOUSEHOLD/PERSON APPLYING FOR ASSISTANCE
