Serves children with qualifying physical disabilities. Provides physical and occupational therapy and equipment evaluations. 

If a child already has CCS medical benefits and is in need of a Medical Therapy Program (MPT)

• Call their case manager at the CCS Admin office.
• Request a review of their records.
