Medicaid recipients that do not own a vehicle and no car tagged in their name.

Application Process
|
Need pre-authorization from the transportation program at the health department. The medical facility requesting transport must call to obtain authorization. Will be granted only for those clients who have a current Medicaid card number and for whom ambulance transportation is medically necessary.

Documents Required
|
Medicaid card and an Ambulance Transfer form. The form must be completely filled out and signed by a licensed physician, and then faxed to the health department.
