Eligibility
|
Must be a PG resident who is currently enrolled in Maryland Medicaid and who meets 1 of the following health criteria and 1 of the following housing criteria:
Health Criteria:
- repeated ER use (meaning more than 4 visits per year) or hospital admissions
- 2 or more chronic conditions, which can include mental health conditions, substance use conditions, physical health conditions, and obesity
Housing Criteria
- will experience homelessness after release from settings such as health facility, mental health facility, substance abuse facility, foster care or other youth facility, or prison
- homeless and at imminent risk of institutionalization
