- Do you have a family history of diabetes?
- Do you have high blood sugar?
- Do you have low or no physical activity daily?
- Do you have high blood pressure or elevated cholesterol?
- Are you American Indian, Hispanic Latino, African American, or Asian American?
- Do you have a previous diagnosis of gestational diabetes?
Please note: This assessment is for those who do not currently have a diagnosis of Type 1 or Type 2 diabetes.
