Description
|
COMMUNITY OUTREACH & WELLNESS: Provides home safety inspections, and assists with filling out advance directives that state how a person's medical care should be handled if they are not able to participate fully in conversations with their care team. Offers blood pressure checks and health screenings, case management for individuals with chronic illnesses (i.e., diabetes, heart failure, COPD, etc.), wellness activities, assists with arranging telehealth consults with healthcare providers, and helps connect people to community resources.

Adults, 18 and older. No services are provided to residents of nursing homes, long-term care facilities, or imprisoned. Undocumented individuals must have a current address in a zip code that the program serves.
