|
SERVICE COORDINATION: Assists pregnant and postpartum women with newborns up to 2 years of age who are considered at-risk due to psycho-social and/or physiological factors. Designed to remove the barriers to medical instruction compliance. Offers prenatal and pediatric coordination and facilitation and case management. Nurses make home visits to monitor the care and progress of expectant mothers and/or infants. Parents' questions are answered and information is given regarding the effects of smoking, alcohol and drugs; postpartum depression; parent/child relationships; preparing for labor and delivery; and immunizations. Refers clients to substance abuse counseling and treatment. Case management provided to help families who have experienced SIDS (Sudden Infant Death Syndrome) and have been referred by The Division of SIDS Study at University of Maryland. Will also refer to Baltimore City Healthy Start, Inc. (see seperate listing).

Pregnant women under 28 weeks, first time mothers, postpartum women and newborns up to 3 years of age who are considered at-risk due to psycho-social and/or physiological factors.
