Description
|
HOUSING: Provides supervised living arrangements in a family setting for Maryland residents age 18 and over, diagnosed with a physical, medical, or mental disability who are in need of protective housing. Case management is provided to adults in care to foster independence and provide safety using an Adult Foster Family Model of Care. Includes a 90 day RN consult component. Recruits and provides clinical support to CARE providers who are certified with the Project Home/Adult Foster Care program.
