Description	EARLY INTERVENTION SERVICES: Provides early intervention services for children ages birth to kindergarten who are suspected of having developmental delays or disabilities. Includes assessment, case management, audiology, physical therapy, speech-language pathology, psychological observations, and a transitional plan for help after graduating from program. Offers a lending library of books and educational materials. Notifies parents of conferences and workshops.

Documents Required	Accepts referrals from parent, health, childcare or social service provider or hospital/NICU staff.
