CASE MANAGEMENT: Family Support Workers help children to be more present in learning through bridging the gap between the school and parents, and connecting with families to address concerns regarding academics, attendance, child behaviors, or the need for family support. This service is provided through a preventative, strengths-based, family driven case management model with contacts made in homes, at schools, and in the community.

NOTE: Must be a Washington County Elementary School student and be referred by a specified school referral source
