Description
|
CASE MANAGEMENT: Provides case management services to adults and senior citizens ages 18 to 64 that are mentally, physically, or developmentally disabled. Assists eligible adults to continue to live full and independent lives in the community. Adults may also be referred to other agencies for financial resources to avoid eviction and utility shut-off.
